For the past year, our computer repair shop has been using a service form made in Word. I want to upgrade to customized service tags, where a customer fills out their data and after everything is explained to them, they'll get a numbered tag - and when they come back to pick up their computer, they just show the tag and we'll match it to the computer, etc....
Is there somewhere local that does this, or do I have to order one from the mainland?

Adrain, Every company has a Purchasing Department where all you have to do is submit your request. Your job is to "fix" computers. Focus on fixing and let the upgrading be done by your supervisors!
Which job is this? The first one or the new one?

Check the mega office stores or the any printing company. I think we've been using a standard multi-part service tag that's had the company name and a service tag number printed on.
While the tag's layout isn't a perfect fit, it's cheaper then going with a completely custom form.
