Re: Bad Promotional Ideas
After Oprah announced a KFC giveaway on her much-watched program, the food chain couldn't keep up with the lines of customers — which led to sit-ins and riots.
At least the mainland KFCs tried to honor that promotion, unlike the Hawaii chain that flat out reneged on the deal. No class for Kazi Foods.

My personal favorite promotional screw-up was the Pepsi Stuff TV commerical, which displayed a Harrier fighter jet that would be awarded to anyone with 7 million Pepsi points. Of course, it was supposed to be a joke. But with no disclaimer saying so, a college student accumulated and purchased 7 million points and actually tried to redeem it for the jet, which led to a messy lawsuit.

But all these promotions were either stupid or poorly planned. The worst promotions are the ones that end up killing people. One that comes to mind is the radio station that held a "Hold Your Wee For a Wii" contest, that resulted in a 28 year old mother of three dying of water intoxication.

In the world of sports, the promo disaster everyone cites as one of the worst was Disco Demolition Night at Comiskey Park in Chicago. But I'm disturbed! I'm depressed! I'm inadequate! I GOT IT ALL! (George Costanza)

Re: Bad Promotional Ideas
There's the "Hands on a Hard Body" contest, replicated across the country, where the person who can keep their hands on a truck the longest wins the truck. There was a great documentary film about the practice. The specific contest in question in Texas was stopped after a participant who didn't win committed suicide.
If you've the time, there's a segment in a recent episode of This American Life on the contest (originally broadcast in 1997, but updated a bit last year). You can listen online.
