In my experience, in VietNam, the military generally laid the blame for any 'mistake' at the feet of the lowest ranking officer, deserved or not. Command officers learned early to CYA, and let the shit roll downhill.

Unfortunately, battlefield reports are either or written or edited by higher-ranking officers, and so the Truth is often obscured, or lost.

My earliest memory of anything military was that the NAVY lied for at least a decade about my uncle's death, suposedly by food poisoning if that's even to be believed, while coming home once WW2 ended. Nothing's changed for the better, our upper echelon brass are mostly corrupt and don't serve this country well at all.
