Hukilau Cafe from 50 First Dates. Although I imagine it to be sort of like Kainoa's Pancake House without the bar part. Has anyone else ever been here? (...) Total hole in the wall and not too kind to tourists.

I haven't seen "50 First Dates" but I've eaten at Hukilau Cafe in Laie many dozens of times. See the photo below. Is that the one from the movie?
And I assume you mean that Kainoa's is rude to tourists, because the staff at Hukilau is exactly the opposite. Lots of visitors to BYU eat at Hukilau since it's just a couple of blocks away, and the tourists are more than welcome there.
So, is this the same Hukilau Cafe from the movie? :
